You are so misled, most drug addicts etc. are very poor and would qualify for medicaid. The people hurt will be the working poor too much money for medicaid and too little to pay for insurance. These are the people who are in the vast wasteland of medical care. As far as insurance companies are considered every single one of them should be exterminated as the vermin they are. The only thing that will reduce your health care costs is some form of national health care or having every single person who can’t pay up front in cash allowed to die in the street with no care. Para medics show to an emergency and either your insured or have a few hundred thousand to pay up front in cash. As we all know many hospitals have over 50% payment delinquency so we have to make a choice. To my way of thought reducing our medical care to below that of most 3rd world countries is not an option. You can’t have it both ways as nothing comes for free. By the way being overweight, smoking, and sedentary life styles cost way more than the  drug addicts. Lower costs by having non emergencies going to clinics, preventive medicine, checklists in all hospitals, dieticians, and fitness training not by throwing the sick to through gutter.
